About Your First Visit
We want your first visit to be as pleasant as possible and we want you to know what to expect. We want our patients to be treated in a warm and compassionate manner. When you visit us for the first time, we will take your medical history and do a cardiac evaluation. Please bring any medicines you are taking and any pertinent cardiac tests your family physician has conducted. This may reduce the amount of testing we need to perform. Please bring your insurance cards. If you have an insurance plan that requires a referral form, please be sure that you bring the referral from your primary physician on the day of the appointment.
Please download, print and complete New Patient Registration, Health History and read and sign the Health Communication Authorization Record. Bring these 3 completed forms with you at the time of your visit.
Pre & Post Operative Instruction form
Prescription refills are issued Monday through Friday between 10:00 a.m. and 5:30 p.m. Please contact your pharmacy and have the pharmacist call our office for the refill. If you need a written prescription, please call us directly.
If you need medical information supplied to your employer, insurance company, or another third party, please send a written request to our office. For your protection, we must have your signed permission to release records.
